Patient Information

June 2, 2020

To all our patients and families,

We hope this letter finds you and your family in good health. We resumed office hours on June 2, however due to the current pandemic we continue to follow guidelines of both the Centers for Disease Control and New York State Health Department.

We now have regulations in place to help protect patients and staff. This includes but is not limited to:

  • Our office will communicate with you beforehand to ask some screening questions. You'll be asked the same questions again when you are in the office.

  • We have hand sanitizer that we ask you to use when you enter the office.

  • Our waiting room no longer offers magazines, children's books/toys etc. for these items are difficult to clean and disinfect.

  • We will ask you to please refrain from bringing any additional family members with you, unless they also have an appointment or they can wait in a car.

  • We respectfully ask that you wait in your car until your appointment time. We will call you or text you when it is time to come in.

  • We ask that you wash or sanitize your hands as often as possible.

  • We love seeing you, but for now a smile and a wave will have to replace a handshake or hug.

As always, if you are feeling sick, have a fever over 100.4, cough, shortness of breath, loss of taste/smell, or any flu-like symptoms, please stay home until you feel better, and call your physician. If you have been traveling to any country identified by the CDC, we recommend that you re-schedule your appointment at least 3 weeks from the time you returned, please check the CDC website at coronavirus.gov.

Infection control has always been a top priority for our practice. Our office follows infection control recommendations made by the American Dental Association, the Centers for Disease Control and Prevention, and the Occupational Safety and Health Administration. We make sure that our infection control procedures are current and adhere to each agencies recommendations.

We look forward to seeing you again, and are happy to answer any questions that you may have. We are doing all that we can to keep our community, patients and team members healthy and safe.

Insurance & Costs

Insurance

Dental insurance is intended to cover some, but not all of the cost of your dental care. Most plans include co-insurance, a deductible and other expenses which must be paid by the patient.

If you have dental insurance, please bring your plan information and insurance card to your first visit. We will work with you to assure that you receive the maximum benefits to which you are entitled. As a service to you, we will file most of your insurance claims for you. This is done by our staff daily in order to expedite payment on claims.

We are currently participating providers for Excellus Blue Cross/ Blue Shield and its contracted affiliates. We also serve a great number of patients who see Dr. Sweeney out of network. Please call us to see how we can help you.

Payment Options

Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office only in advance of starting your treatment.

For your convenience, we accept cash, personal checks, money orders, Visa, Discover and Mastercard. Financing is available with Care Credit.
